The purpose of this document is to outline the terms and conditions under which “Family Enrichment Through Computing and Hope, Inc.” (The F.E.T.C.H. Program) will operate. The details of said document hereby constitute a legal and binding contract between the undersigned parent/guardian/student/applicant and The F.E.T.C.H. Program. It is absolutely necessary for each line-item to be initialed by/for the student/applicant. Any failure to initialize one or more line-item renders the contract incomplete. No student/applicant will be admitted to this program with an incomplete contract, regardless of that individual student’s circumstances/justification.

A signature of the student/applicant and his or her parent/guardian is necessary in order to consummate this agreement. Once this agreement is signed, it is understood that the parent/guardian/student/applicant agrees to comply with the rules and regulations laid out in the details of each initialed items and further pledges their full commitment to honoring the spirit of said agreement, recognizing the relevance of this contract towards the successful operation of The F.E.T.C.H. Program.

This venture is the “Family Enrichment Through Computing and Hope, Inc. Program”, and shall be referred to from here forward as “The F.E.T.C.H. Program”. The provider of services for said venture; Barrett Information Technologies, Inc. shall be referred to from this point forward as “The Service Provider”. 

Failure to comply with any aspect of the outlined program details below constitutes a direct breach of this contract, and hereby renders student ineligible for program participation and will result in immediate, unavoidable termination of services offered to said student, and immediate removal/dismissal from The F.E.T.C.H. Program, regardless!

Contractual Agreement:The F.E.T.C.H.ProgramParticipantsContractfor the applicant

Item 1: Attendance and Tardiness.

  1. The Service Provider will conduct classes/offer training on Computer Assembly, Software Development, Web Development, Graphics, Game Development and Animation.
  2. All students/participants are expected to attend all classes.
  3. Classes will begin on _______, 2020 and end _______, 2020.
  4. Classes will be held on Tuesdays and Thursdays and will begin at begin 6 pm sharp, and end at 8:30 pm.
  5. A light meal will be served at 7 pm.
  6. Attendance will be recorded for each class.
  7. Roll-call ends at 6:05 pm.
  8. Students are considered tardy after 6:05 pm.
  9. Participants will be allowed a maximum 2 incidents of tardiness.
  10. 3 incidents of tardiness constitute a violation of this contract, and as such provide grounds for immediate dismissal from The F.E.T.C.H. Program.
  11. Participants will be allowed one excused absence.
  12. More than one absence constitutes a violation of this contract and provides grounds for immediate dismissal from The F.E.T.C.H. Program.

Item 2: Discipline

The F.E.T.C.H. Program assumes that all parents/guardians/students/applicants understand the importance of the technology training offered by The Service Provider towards the improvement of their own personal lives and livelihood. The Service Provider hereby stresses that The F.E.T.C.H. Program and its benefits are been offered to parents/guardians/students/applicants at no cost, however, The F.E.T.C.H. Program and its benefits are by no means free.

The Service Provider therefore STRONGLY mandates the following simple rules and vows harsh repercussions if such rules are not adhered to:

All parents/guardians/students/applicants MUST conduct themselves in an orderly manner in order to be considered qualified/eligible to receive the training and benefits offered by The F.E.T.C.H. Program.

Under No circumstance will there be any allowance made for insubordination, disrespectful attitude, fowl language, disruptive conduct or lewd behavior. Inappropriate chatter, side-shows, and other distractions MUST be checked at the door.

Item 3: Program Quality and Dress Code.

In an attempt to maintain the highest standards possible as well as securing a healthy future for The F.E.T.C.H. Program, a Dress Code Policy, conducive to the establishment of an ethically, professionally, physically, and mentally sound demeanor has been adopted, and is outlined as follows:

  1. All parents/guardians/students/applicants are expected to carry themselves in a professional manner, consistent with that of technicians employed in the aspired to technology related positions.
  2. Because of the nature of The F.E.T.C.H. Program, and the assumption on the part of The Service Provider that parents/guardians/students/applicants appreciate the tremendous blessings afforded to them by being allowed to participate in The F.E.T.C.H. Program, The Service Provider assumes that each parents/guardians/students/applicants will see the need to act as partners in helping to ensuring the maintenance of high standards in the quality of our instituted Dress Code Policy, and the proper conduct of participants.
  3. All parents/guardians/students/applicants are hereby deputized to assist Instructors and Administrators in this ongoing effort to ensure the safety of equipment and supplies as well as the general upkeep of the building, room and furniture used by The F.E.T.C.H. Program.
  4. Etchings, Graffiti, defacing of furniture or any other acts of vandalism is strictly prohibited.
  5. It is a foregone conclusion that all male parents/guardians/students/applicants will wear belts in their pants, where necessary, and, at the very minimum, a shirt and a pair of closed-toes shoes in order to be considered eligible to participate in The F.E.T.C.H. Program.
  6. No Tank-Tops/Wife-Beaters allowed.
  7. In the same token, all female parents/guardians/students/applicants are expected to wear decent attire and closed-toes shoes in order to be considered eligible for The F.E.T.C.H. Program.
  8. Any form of indecent attire provides grounds for immediate dismissal from The F.E.T.C.H. Program.
  9. For the purpose of this contract, indecent attire is hereby described as follows: Any clothing item allowing for excessive exposure of body parts, example midriff shirts/blouses, Low-rider Jeans, Daisy Dukes or Booty-Shorts, Low-Cut Shirts or Blouses and any other clothing item which falls in said category.

Item 4: Computer Giveaway.

  1. The F.E.T.C.H. Program is intended for family members and as such one family could possibly constitute multiple siblings. Each enrolled family, however, will be allowed to take home ONLY one computer system on which they worked throughout the duration of this program, regardless of the number of siblings or family members participating in The F.E.T.C.H. Program.
  2. All computers worked on throughout the duration of The F.E.T.C.H. Program remains the property of The F.E.T.C.H. Program, and as such may not be removed from the premises until the program ends on __________, 2020.
  3. On________, 2020, The F.E.T.C.H. Program will award each participating family, which completes all assigned coursework and complied with The F.E.T.C.H. Program requirements, one complete computer system (computer, monitor, keyboard and mouse, NO PRINTERS).

Item 5: Program Participation.

  1. The Service Provider and designated instructors will assign class work which must be completed in each class period in order to satisfy the requirement for class participation. Failure to participate in class constitutes a breach in this contract and therefore provides grounds for dismissal from The F.E.T.C.H. Program.
  2. Each student/applicant is expected to be represented by a parent/guardian/caregiver in the F.E.T.C.H. Mom/Dad Volunteer Program, wherein said persons are each provided with a list of volunteer opportunities available to them for the duration of the student/applicant participation in the program.
  3. Failure, on the part of the parent/guardian/care-giver to participate in at least one The F.E.T.C.H. Program volunteer opportunity constitutes a breach of our protocol and therefore represents a reason for dismissal of said student/applicant from The F.E.T.C.H. Program. No exception!

Item 6. Participants Agreements:

  1. As a parent/guardian/student/applicant I agree to maintain respect, decency and order while participating in The F.E.T.C.H. Program.
  2. I understand that The F.E.T.C.H. Program Instructors will NOT be allocating precious class time to dealing with discipline issues.
  3. I understand that The F.E.T.C.H. Program Instructors would rather deal with disciplined students as this would help us to achieve the very ambitious program goals and objectives, set forth by The F.E.T.C.H. Program.
  4. I understand that The F.E.T.C.H. Program Instructors and Administrators will move expeditiously to make examples of violators in order to minimize distractions, while they focus on providing services to the better adjusted participants.
  5. I agree to abide by he rules and regulations outlined above. I also understand that in the event that I violate the spirit, or details of the contract in any way, shape or form, then I will be dismissed from The F.E.T.C.H. Program immediately.
